Each of you comes here today hopeful, wanting in on the game. A month ago you were in med school being taught by doctors. Today, you are the doctors. The seven years you spend here as a surgical resident will be the best and worst of your life. You will be pushed to the breaking point. Look around you. Say hello to your competition. Eight of you will switch to an easier specialty. Five of you will crack under the pressure. Two of you will be asked to leave. This is your starting line. This is your arena. How well you play? D（Philosophy of Doctoral）学位，即哲学博士，或者同时获得MD和Ph.D。获得MD后，如果有志于从事临床工作，就向一家医院提出当住院医生（resident） 的申请，获准后，开始 resident 生涯。至于 residency program 有几年，每个医院的规定是不同的，哈佛医学院（Harvard Medical School）的麻省总医院（Massachusetts General Hospital）的外科住院医生计划（Surgery Residency Program）是5年，而在 Grey's Anatomy 中，假想的 Seattle Grace Hospital 的 Surgery Residency Program 是7年（它自诩为除哈佛医学院外最严格的外科住院医生计划）。通过 Residency Program 便可以成为某一科的专科医生，如外科医生，内科医生等。第一年的住院医生（resident）被称为实习医生 （intern），就是 Meredith 这班菜鸟在第一季的地位。一般，interns 可以由高几年的residents 来带，就是 Grey's Anatomy 中 'Nazi' Miranda 的角色，而 residents 由主治医生（attending doctors）来监督，Grey's Anatomy 中的 Dr. Shepherd 和 Dr. Burke，attending doctors 则个个觊觎主任医生（chief）的位置，Grey's Anatomy 中的大家长 Dr. Webber。
